Click here for the Refer a Member form. The new member must open a prestige (S5) or premium (S4) account, enroll in online Bill Pay, or sign up for direct deposit. The for office use section should be completed by the MSR opening the account and then it should faxed to the accounting department. The accounting department will deposit the $50 to the referring member account after the account is open for 90 days and the qualifications are met.
